Checks are due one week prior to tournament start date.

You must have a copy of a birth certificates for all children on your roster in case of protest.

All teams must carry their own insurance and provide proof of it prior to the tournament. Exception: If the manager can show proof of Insurance/release that every child on the roster has insurance and will not claim any injury against Katy American and the Summer Sizzler staff that will be acceptable also. If a child does not have medical insurance that child will not be eligibale for game play. If you carry this kind of waiver it must state that Katy American and the Summer Sizzler staff are not responsible for anything including Co-pays and Etc. You must provide the Summer Sizzler medical release and your own. This must be turned in at check-in time.

Official rosters are due before your first game.
